7-6 Academic Course Forms
The forms found in this section are to be used for the academic course additions, deletions and revisions. The other portions of an academic course proposal do not have specific forms, but should be included in the proposal. Refer to "Format for Academic Course Proposal" in Appendix B—Resources for Curriculum Proposals.
- Curriculum Proposal Routing Slip must have the signatures of associate dean(s), registrar(s), and either or all of the following: academic dean and technical dean on Central, vice president of instruction on North, dean of instruction on South.
- Curriculum Proposal Form signed by the department chair and curriculum facilitator. Instructions for the completion of this form are included in this section.
- Course Maintenance Inventory Form is needed to enter the course into the computerized inventory of courses. Instructions for the completion of the form are included in this section.
- Catalog Course Description is an internal document. It is necessary for catalog revisions. When choosing a course from either ACGM or WECM, submit a photocopy of the rubric/course number/contact hours and CIP to the registrar together with the curriculum proposal. These are not to be included in the proposal, but they can shorten the time needed for the registrar’s review and signature.
