7-6 Unique Need Course Forms
In addition to the forms required for academic courses in the previous section, the forms found in this section are necessary for a unique need course. These forms will be submitted to THECB.

  2. Unique Need Course: Request for Approval Form: Instructions for the completion of this form are included in this section. The form must be signed by the Chief Academic Office—that is, vice president of instruction.
